The Cotton Creek Membership is an annual reduced-fee savings plan for individuals and families that allows all Cotton Creek Members to receive quality dental services at greatly reduced prices. Unlike conventional insurance plans, there are no deductibles, no yearly maximums, and no waiting periods to begin treatment.

Note: The Dual Plan is for parent/child or husband/wife. The Family Plan includes up to six family members (children until age 19). Additional family members are $95 each.
